  • You can fill in the PDF on your device or you can print the form
  • Complete the form for each door on your premises (Continuation sheets can also be downloaded as required)
  • If you fill in the PDF on your device then you can simply Save it as a new file and email it to safety@fingersafegb.com
  • Or if you’ve printed the form then scan and email the completed forms to safety@fingersafegb.com

Please remember to keep a copy of your risk assessment for your health and safety folder.
